Why your knee replacement might fail #kneereplacement #surgery
Description: Why would a better-looking knee do worse after knee replacement?
Most of the time, we guide patients not to let their X-ray grade make the decision. There's one place where the grade can changes our surgical advice: when someone who is not yet bone on bone wants a knee replacement.
Published data: In a study of nearly 1,000 total knee replacements, Schnurr and colleagues found that #arthritis severity before surgery was the only factor that predicted satisfaction afterward. Compared with patients who had end-stage, grade 4 arthritis, the risk of dissatisfaction was up to 3X higher with grade 2. Polkowski and colleagues similarly found that unexplained pain after knee replacement was more common in patients whose arthritis was early-grade before surgery.
Why? When the joint is worn through, replacing it removes the main source of pain, and the improvement is dramatic. When arthritis is milder, other pain generators may be contributing: tendinitis, meniscal problems, inflammation, even referred pain from the hip. A new joint does not fix those. Expectations matter too. Reviews of patient satisfaction consistently identify unmet expectations as one of the strongest predictors of disappointment after surgery. When a patient with milder arthritis is considering a replacement, we have a deliberate conversation first. Are we sure nothing else is driving the pain? Those sources need to be found and treated before surgery - to protect the result.
