The point of your elbow is one of the most exposed bones in your body - with minimal padding. Your #olecranon is also where the triceps attaches. When it breaks, that muscle pulls the fragment up the arm and opens the fracture like a hinge. Because the break runs into the joint surface, most displaced olecranon fractures need surgery to restore the joint and get the elbow moving again.
For decades the standard repair was tension band wiring: two pins + a figure-of-eight wire that convert the triceps pull into compression across the fracture - fast and effective for simple transverse breaks. The tradeoff can be hardware that sits directly under the skin and often has to come out later. Pre-contoured plates offer an alternative. They sit on the back of the ulna (tension side) and wraps over the tip of the elbow. Proximal screws are angled into the dense bone at the base of the coronoid while stopping short of the joint surface. Many constructs add one long screw from the olecranon tip down the canal of the ulna (called the home run screw). The goal is solid purchase on both sides of the fracture, typically at least three screws above and three below.
Both techniques can deliver excellent function and the right choice depends on fracture pattern, bone quality, soft tissue condition, and patient goals.
