Spinal or general anesthesia for hip and knee replacement? It is one of the most common questions I hear in clinic, and the answer is more nuanced than most expect.
Spinal anesthesia is a single injection in the lower back that numbs the body from the waist down. Patients still receive sedation, so most are relaxed and resting during surgery while breathing on their own, without a breathing tube. #spinalanesthesia
For elective joint replacement, observational studies have generally favored spinal anesthesia for early recovery: less postoperative pain and nausea, lower opioid requirements, and smoother same-day discharge in the outpatient setting, with similar complication rates between the two approaches.
For my colleagues, two details are worth highlighting. First, agent selection matters. A shorter-acting spinal agent can shorten recovery room time compared with longer-acting options, a real advantage in the ambulatory setting. Second, the strongest randomized evidence on this question comes from hip fracture surgery in older adults, where spinal was not superior to general anesthesia for survival, walking ability, or delirium. That is a different population than elective arthroplasty, and it reminds us that technique choice should be individualized.
Spinal is not right for everyone. Bleeding disorders, certain blood thinners, infection at the injection site, some heart conditions such as severe aortic stenosis, and certain neurologic conditions may make general anesthesia the safer choice. That decision is made together with your anesthesiologist. #jointreplacement
